Intellectual Disability Annual Health Assessment

Each year, individuals with intellectual disability are eligible for a health assessment covered by Medicare. This assessment is crucial for understanding individuals health needs and ensuring appropriate follow-up care.

The CHAP (Comprehensive Health Assessment Program) is an evidence-based tool for conducting these assessments and is endorsed by the Australian Government.

The CHAP assists with gathering important insights into an individual’s current health, ensures regular screening takes place and allows GPs to gather an individuals health baseline.

CHAP: Part 1 is filled in by the individual and the people who know them best and can speak to their health over the past year.

CHAP: Part 2 is completed by the Practice Nurse and individual’s regular GP.

Support for completing the CHAP

Project GROW’s Annual Health Assessment checklist is designed to assist service providers in preparing an individual and their support team to attend an Annual Health Assessment (AHA) with confidence. The checklist provides an understanding of what is required before, during and after the appointment.

Project GROW provides specialised training for both primary care and disability support providers. This training equips professionals to fully understand and effectively execute their role in the assessment process, facilitate clear communication and implement adjustments that support everyone to have a good appointment.
